Additional Fees:
There is an additional $30 dollar fee for completion of forms for use outside of the patient's medical record, including:
- Day Care
- Pre-Employment
- Employment
- Life Insurance.
- Family Medical Leave (FMLA)
- School
All completion of forms require an office or telemedicine visit.
Cancellations must be made greater than 24 hrs before the office visits, otherwise it is subject to a $50 cancellation fee.
